慢性阻塞性肺疾病患者疾病不确定感与应对方式的相关性分析

目的探讨慢性阻塞性肺疾病(chronicobstructive pulmonary disease,COPD)稳定期患者的疾病不确定感与应对方式之间的相关性,为临床干预提供依据。方法便利抽样法选择解放军第401医院2016年1-8月收治的COPD患者104例,使用中文版Mishel疾病不确定感量表(the Mishel’s uncertainty in illness scale,MUIS)、医学应对问卷(medical coping modes questionnaire,MCMQ)对其进行问卷调查。结果患者疾病不确定感分值范围为46~138分,平均(88.08±16.14)分,单题平均得分为(2.67±0.49)分。处于不确定感低程度的患者12例占11.54%,中等程度者89例占85.58%,高程度者3例占2.88%。疾病不确定感总分及其两因子与"面对"、"回避"应对方式呈显著负相关(P0.05),与"屈服"应对方式呈正相关(P0.05)。结论 COPD患者存在疾病不确定感,而且疾病不确定感与应对方式显著相关,应该引起护理人员的足够重视,在护理患者的过程中应该给予积极的支持和引导,从而降低患者的疾病不确定感。

Correlation between Uncertainty in Illness and Coping Style in Patients with Chronic Obstructive Pulmonary Disease

Objective To investigate the relationship between uncertainty in illness and coping style in patients with COPD,in order to provide theoretical basis of clinical intervention.Methods By convenience sampling method,104 cases were selected and investigated by the Mishel's uncertainty in illness scale (MUIS) and medical coping modes questionnaire(MCMQ).Results The total score of patients'uncertainty in illness was 46-138,(88.08± 16.14) in average and average score for each item was(2.67±0.49).There were 12 patients with low level of uncertainty.accounting for 11.54％.89 patients with medium level of uncertainty,accounting for 85.58 ％,and 3 patients with high level of uncertainty,accounting for 2.88 ％.The score of uncertainty in illness as well as its two factors was negatively related to coping style such as confrontation and avoidance(P＜0.05),and positively correlated to Coping style like surrender(P＜0.05).Conclusions Patients with COPD disease uncertainty in illness is associated with uncertainty and copying style,which required attention from nursing staffs,as well as the active support and guidance,in order to reduce the uncertainty of illness.
