肺大疱的诊断与微创手术治疗

目的 探讨肺大疱的诊断与微创手术治疗的疗效.方法 对45例肺大疱患者实施电视胸腔镜手术,总结应用电视胸腔镜手术治疗肺大疱的临床经验.结果 术前CT发现肺大疱25例,胸部X线片发现肺大疱10例.45例患者均采用电视胸腔镜手术,其中7例行电视胸腔镜辅助小切口手术.手术顺利,术中失血平均约55 ml,手术时间约50～130 min,术后疼痛不适感较轻,无术后长期漏气等并发症.所有患者术后均痊愈出院,随访至今无复发.结论 电视胸腔镜手术创伤小、出血少、安全可靠,是治疗肺大疱首选微创手术方法.胸部CT在术前诊断中具有重要意义.

The diagnosis and minimal-invasive surgical treatment of pulmonary bulla

Objective To investigate the diagnosis and minimal-invasive surgical treatment of pulmonary bulla.Methods Totally 45 patients with pulmonary bulla underwent video-assisted thoracoseopie surgery.All patients underwent preoperative chest radiography scan and 36 patients underwent preoperative chest CT scan.Results All patients were cured.Pulmonary bullae were detected by preoperative chest CT scan in 25 patients, by preoperative chest radiography scan in 10 patients.Conclusions For pulmonary bulla, video-assisted thoracoscopie surgery is the preferred technique of minimal-invasive surgical treatment with minimum trauma, little bleeding, safety.Chest CT scans have important significance in the preoperative diagnosis of pulmonary bulla.
