摘    要:
目的探讨血压分层对臂踝脉搏波传导速度(baPWV)的影响。方法从欧姆龙全自动动脉硬化监测仪存储器中获取行baPWV检测的5643例体检者和门诊高血压患者的资料,比较不同血压分层和不同年龄受试者的baPWVo采用Pearson相关分析评估右侧baPWV与其他变量的相关性,采用多因素线性回归分析、多因素回归分析评估baPWV的影响因素。结果随着收缩压分层的增加,baPWV逐渐增加,差异有统计学意义(P<0.05)。<30岁组、30~39岁组、40~49岁组、50.59岁组、60~69岁组.70-79岁组.80-89岁组、90-99岁组baPWV比较差异有统计学意义(P

Abstract:
Objective To explore the effect of blood pressure stratification on brachial ankle pulse wave velocity ( baPWV ). Methods The data of 5 643 people who took healthy examination and outpatients with hypertension were obtained from Omron automatic artery stiff detector. BaPWV of different blood pressures and ages were compared. Correlations among baPWV of right side and other indexes were evaluated by Pearson correlation analysis. Influencing factors of baPWV were evaluated by multivariate linear regression analysis and multivariate logistic regression analysis. Results With the increase of systolic pressure stratification, baPWV increased gradually and significantly ( P < 0. 05 ). BaPWV among <30 years old group,30-39 years old group,40-49 years old group,50-59 years old group,60-69 years old group,70-79 years old group, 80-89 years old group, 90-99 years old group were significantly different (P < 0. 05). Results of Pearson correlation analysis showed that baPWV of right side was positively correlated with gender, age, systolic blood pressure, mean blood pressure, diastolic blood pressure, pulse pressure, pulse rate, body mass index, hypertension history, diabetes history and hyperlipidemia history (P <0. 05). Results of multivariate linear stepwise regression analysis showed that B value of systolic pressure affecting baPWV was 9. 22 ( P < 0. 001 ). Results of multivariate logistic regression analysis showed that risk of baPWV≥ 1400 cm/s in normal high group Ⅰ, normal high group Ⅱ and hypertension group all increased compared with that in ideal systolic pressure group and their OR value rised with classification of blood pressure increasing. After adjusting age, gender, BMI, hypertension history, diabetes history, hyperlipidemia history and so on, risk of baPWV ≥ 1 400 cm/s in normal high group Ⅰ, normal high group Ⅱ and hypertension group all increased and their OR value still rised with classification of blood pressure increasing. Conclusion The effect of blood pressure on baPWV is obvious. Alter influencing factors are removed, the effect is still existed. It is necessary to propose a normal range of baPWV according to different blood pressure.
