LEEP锥切术对宫颈上皮内瘤变的治疗效果及其对患者宫颈恢复时间的影响

目的探讨LEEP锥切术对宫颈上皮内瘤变的治疗效果及对患者宫颈恢复时间的影响。方法选择医院妇科2015年1月至2017年1月收治的80例宫颈上皮内瘤变患者为研究对象,将患者按随机数字表法分为LEEP组(n=40,LEEP锥切术)与冷刀组(n=40,冷刀锥切术),比较两组的治疗效果。结果 LEEP组手术时间、术后住院时间、切口愈合时间及宫颈恢复时间均短于冷刀组,术中出血量少于冷刀组(P<0.05)。LEEP组手术并发症总发生率低于冷刀组(P<0.05)。两组患者切缘阳性率、18个月内复发率比较,差异无统计学意义(P>0.05)。术后6个月,两组表皮生长因子受体过度表达率、人乳头瘤状病毒阳性率均降低(P<0.05),但两组比较,差异无统计学意义(P>0.05)。术后6个月,两组Barthel指数、SF-36评分均升高,且LEEP组高于冷刀组(P<0.05)。结论 LEEP锥切术治疗宫颈上皮内瘤变近远期效果良好,且创伤小,并发症少,更利于术后恢复,值得推广。

Therapeutic effect of LEEP conization on cervical intraepithelial neoplasia and its influence on cervical recovery time

Objective To investigate the effect of LEEP conization on cervical intraepithelial neoplasia and its influence on cervical recovery time.Methods Eighty patients with cervical intraepithelial neoplasia admitted in gynaecology department of our hospital from January 2015 to January 2017 were selected as study objects.The patients were divided into LEEP group(n=40,LEEP conization)and cold knife group(n=40,cold knife conization)by random number table.The therapeutic effects of the two groups were compared.Results The operation time,postoperative hospital stay,wound healing time and cervical recovery time in the LEEP group were shorter than those in the cold knife group,and the intraoperative bleeding volume was less than that in the cold knife group(P<0.05).The total incidence of operative complications in the LEEP group was lower than that in the cold knife group(P<0.05).There were no significant differences in the positive rate of incision margin and recurrence rate within 18 months between the two groups(P>0.05).At 6 months after operation,the overexpression rate of epidermal growth factor receptor and positive rate of human papillomavirus decreased in both groups(P<0.05),but there were no significant differences between the two groups(P>0.05).At 6 months after operation,the scores of Barthel index and SF-36 in both groups increased,and those in the LEEP group were higher than the cold knife group(P<0.05).Conclusion LEEP conization has good short-term and long-term effects in the treatment of cervical intraepithelial neoplasia,with less trauma and fewer complications,which is more conducive to postoperative recovery and is worth promoting.
