超声对冠状动脉介入术后急性桡动脉闭塞的诊断价值

摘    要:目的 探讨超声对冠状动脉介入术后桡动脉闭塞的诊断价值.方法 选取经桡动脉冠状动脉介入术后桡动脉急性闭塞患者34例(桡动脉闭塞组),以健康体检者34例为对照组.应用超声检测介入术后闭塞桡动脉术前内径,并与对照组进行比较;同时比较分析触摸桡动脉搏动法与超声诊断法在评估桡动脉闭塞的诊断准确性的差异.结果 桡动脉闭塞组术前桡动脉内径为(2.03±0.25) mm,小于对照组(2.48±0.44)mm,两组比较差异有统计学意义(P<0.001).桡动脉闭塞组中,经临床触摸脉搏法发现20例桡动脉无搏动,诊断率为58.82%;超声诊断桡动脉闭塞34例,诊断率为100%,后者高于前者,差异有统计学意义(P<0.001),其中5例可见细小血流来自侧支循环.结论 超声检查更有利于对冠脉介入后桡动脉闭塞的诊断.

Abstract:Objective To investigate the diagnostic value of high-frequency ultrasonography on acute occlusion of radial artery after percutaneous coronary intervention. Methods After percutaneous coronary intervention via the radial artery, 34 patients with acute occlusion of radial artery and 34 healthy subjects were enrolled. The internal diameters before the intervention in patients with post-interventional occlusion of radial artery were detected by ultrasonography,and they were compared with the control group. Differences between touching pulse and ultrasonography in the diagnostic accuracy of assessing the occlusion of radial artery were compared. Results The internal diameter of radial artery before the intervention in occlusion group was(2.03±0.25)mm,and it was(2.48±0.44)mm in control group. There was a significant difference between the two groups (P<0.001). In occlusion group,20 cases of radial artery without pulse were detected by clinical touching pulse method. The diagnosis rate was 58.82%. Thirty-four cases of radial artery occlusion were diagnosed by ultrasonography. The diagnosis rate was 100% ,which was higher than the former,the difference was statistically significant (P<0.001). Small flow from collateral circulation was found in 5 of 34 cases. Conclusion Ultrasonography is more conducive to the diagnosis of radial artery occlusion after coronary intervention.
