对侧预防性乳腺切除术在散发性乳腺癌中的应用现状

对侧预防性乳腺切除术（CPM）可降低对侧乳腺癌的发生率,并提高BRCA基因突变乳腺癌患者的生存率,但对早期散发性乳腺癌生存率的影响仍存在争议.近年来CPM的应用逐渐增多,与乳房重建术的实施、基因测试、医师决策等有关.在激素受体阴性的患者中,CPM提高了无病生存率,但现有的研究无法证实其对总生存的影响,CPM的选择须谨慎,其指征可能包括年轻、早期、激素受体阴性患者或患者强烈意愿等.

Contralateral Prophylactic Mastectomy in Women with Sporadic Breast Cancer

Although contralateral prophylactic mastectomy （CPM）could reduce the risk for developing contralateral breast cancer and mortality of breast cancer patients with BRCA gene mutation, it is still contro-versial whether it reduces mortality of early sporadic breast cancer. The reason of increasing in CPM rate recently can be related to the implementation of breast reconstruction,gene test, and doctor＇s decision etc.. Although CPM is proved to have reduced breast cancer-related mortality in hormone receptor negative patients,its effects to overall survival remains uncertain. Choice of CPM should be careful, the indications may include ： young age, early stage, negative hormone receptor or patient＇s insisted preference.
