强化降脂对缺血性脑卒中患者炎症因子的影响及疗效观察

目的探讨强化降脂对缺血性脑卒中患者炎症因子的影响及疗效观察。方法将80例缺血性脑卒中患者随机分为强化组和普通组。两组患者予以控制颅内压、控制血压血糖、抗血小板聚集、改善脑局部微循环等常规对症治疗，普通组在此基础上加用阿托伐他汀20mgqd口服，强化组在此基础上加用阿托伐他汀40mgqd121服，连用4周。结果治疗8周后，两组患者血浆hs—CRP、IL-8和TNF-α水平均较治疗前明显下降（P〈0．05或P〈0．01）．且强化组下降的幅度较普通组更显著（P〈0．05）；强化组的临床总有效率明显高于普通组（X2=6．27，P〈0．05）。结论阿托伐他汀强化降脂治疗缺血性脑卒中的疗效肯定，通过改善血管内斑块的局部炎症反应，产生抗炎作用，从而达到改善梗死部位缺血缺氧和缺损神经功能的作用。

The influence of intensive lipid lowering therapy on the inflammatory factor in paitents with ischemic stroke

Objective To explore the influence of intensive lipid lowering therapy on the inflammatory factor in patients with ischemia stroke.Methods Eighty patients with ischemia stroke were divided into intensive group and traditonal group randomly.All the patients got treatment of intracranial pressure control,blood pressure and sugar control,platelet aggregation inhibition and cerebral microcirculation improvement.The traditonal group got extra treatment as taking atorvastatin orally 20 mg per day,and the intensive group got extra treatment as taking atorvastatin orally 40 mg per day,and all of the treatments lasted for 4 weeks.Results Eight weeks after treatment,the plasma levels of hs-CRP,IL-8 and TNF-α of either group decreased significantly compared that before treatment(P < 0.05 or P < 0.01),and the change was more obviously in intensive group(P < 0.05).Meanwhile,the clinic effective rate of the intensive group was significantly higher than that of traditional group(χ2=6.27,P < 0.05).Conclusion Treating cerebral ischemic stroke by intensive lipid lowering therapy using atorvastatin shows a certain clinic curative effect.Maybe it could improve the local inflamation of the vascular plaques,thus lead to improve the ischemia and anoxemia of the cerebral infarction area and the function of the defect nerves.
