三种不同麻醉方法对腹腔镜下胆囊切除术中应激反应及血气变化的影响对比观察

目的评价三种不同麻醉方法对腹腔镜下胆囊切除术中应激反应及血气变化的影响,对比各种方法的疗效。方法选择2010年1月至2012年10月于我院就诊行腹腔镜下胆囊切除术的150例患者的临床资料进行回顾性分析,随机抽取全麻组56例、硬膜外麻醉组52例,全麻复合硬膜外麻醉42例,观察麻醉前后、气腹后10min时心率、平均动脉压、C反应蛋白、PETCO2、SpO2各项指标,并运用分析各关指标。结果全麻复合硬膜外麻醉患者在术中心率、平均动脉压、C反应蛋白等各项应激反应指标及PETCO2、SpO2指标较单纯全麻及单纯硬膜外麻醉改变比较,差异有统计学意义（P〈O．05）。结论全麻复合硬膜外麻醉下,患者的术中应激反应较小,生化指标改变波动较小。

Impacts of three different anesthetic methods on stress response and blood gas changes during laparoscopic cholecystectomy

Objective To explore the impacts of three different anesthetic methods on stress response and blood gas changes during laparoscopic cholecystectomy. Methods The clinical data on 150 patients who had undergone laparoscopic cholecystectomy in our hospital during the period of January 2010 to December 2012 were retrospectively analyzed. 56 patients were in the group of general anesthesia, 52 werein the group of epidural anesthesia, and 42 were in the group of general anesthesia combined with epidural anesthesia. Heart rate, mean arterial pressure, C-reactive protein, PETC02, and Sp02 were observed before and after anesthesia and lOmin after formation of pneumoperitoneum, l{eoultg Heart rate, mean arterial pressure, C-reactive protein, PETC02, and SpO2 differed significantly between the group of general anesthesia combined with epidural anesthesia and the group of general anesthesia or epidural anesthesia alone （P〈0.05）. Golmlugiong Under general anesthesia combined with epidural anesthesia, patients develop less stress response and have less fluctuation in biochemical indicators.
