生长抑素对大鼠坏死性胰腺炎GH-IGF-1-白蛋白轴的影响及其对肠黏膜屏障的保护作用

摘    要:目的:研究生长抑素(somatostatin,S)对坏死性胰腺炎(acute necrotizing pancreatitis,ANP)肠黏膜屏障损害的治疗作用.方法: 逆行胰胆管注射3.5%牛磺胆酸钠(0.2 ml/100 g)诱导ANP模型.分四组,分别为正常( N)组、假手术(SO)组、坏死(ANP)组和治疗(ANP+S)组.术后24 h抽血测血浆D-乳酸、血清G H、胰岛素样生长因子(IGF-1)和白蛋白浓度,取末端回肠观察并计算绒毛高度和黏膜厚度变化. 结果ANP组回肠通透性上升,绒毛高度和黏膜厚度减小,血清白蛋白、GH和IGF-1均下降.生长抑素治疗后可减轻ANP鼠肠黏膜厚度及绒毛的改变,降低肠道通透性,血清GH、白蛋白浓度较ANP组为高,IGF-1两组无显著性差异. 结论外源性生长抑素对ANP鼠肠黏膜屏障有保护作用.

Abstract:Objective:To investigate the effect of somatostatin(S) on the intestinal mucosal barrier dysfunction in acute necrotizing pancreatitis(ANP) rats.Methods:ANP rats were induced by retrograde infusion of 3.5% sodium taurocholate into biliary pancreatic duct,the rats were divided into 4 groups:normal(N),sham operation(SO),ANP and treatment(ANP+S) group. Plasma D lactate,serum GH,insulin like growth factor 1(IGF 1) and albumin level were measured in 24 hours postoperatively. The height of villi and the changes of the mucosal thickness at the terminal end of ileum were observed and calculated.Results:The gut permeability of ileum in ANP group raised and the height of villi and the mucosal thickness decreased. The level of the serum albumin,GH and IGF 1 all lowered. Treatment with somatostatin could relieve the changes of the mucosal thickness and height of villi and lowered the gut permeability. The concentration of serum GH and albumin of the treatment group was higher than that of ANP group. There was no significant difference between IGF 1 and treatment group.Conclusion:Exogenous somatostatin has protective effect on the intestinal mucosal barrier of ANP rats.
