经颈静脉肝内门体分流术治疗肝硬化门脉高压症的效果分析

目的探讨经颈静脉肝内门体分流术（TIPS）治疗肝硬化门脉高压症的临床效果。方法追踪随访在我院因静脉曲张破裂出血而接受TIPS治疗的肝硬化门脉高压症患者42例．观察治疗效果。结果42例患者均成功实施TIPS。所有患者随访12-24个月，其中腹水消退总有效率为83．33％，术后24小时止血率为88．10％，肝性脑病发生率为7．14％，再出血复发率为9．52％。多普勒超声示支架狭窄或闭塞发生率为11．90％。治疗后患者门静脉主干以及脾静脉内径、门脉主干压、门脉血流速度均较治疗前改善，两者比较差异具有统计学意义沪〈0．05）。治疗后患者ALT、AST、ALB及TBIL与治疗前比较差异无统计学意义（P〉0．05）。结论TIPS治疗肝硬化门脉高压症效果确切．安全可靠．值得应用．

Analysis on Effect of Transjugular Intrahepatie Portosystemic Shunt in Treating Portal Hypertension about Cirrhosis of Liver

Objective To invesgate the effects of transjugular intrahepatic portosystemic shunt（TIPS） in treating portal hypertension about cirrhosis of liver relations. Methods 42 cases with portal hypertension about cirrhosis of liver who received TIPS were followed-up and the effects were observed. Results The 42 cases received TIPS successfully. All of patients were followed-up about 12 to 24 months.The total effective rate ascites regression was 83.33%, rate of hemostasis in 24 hours was 88.10%, incidence rate of hepatic encephalopathy was 7.14% and the relapse rate of rehaemorrhagia was 9.52%.The relapse rate of flame stenosis or obstruction was 11.90 %. The inside diameter of main portal vein and splenic vein, blood pressure and blood flow rate of portal vein after operation improved more than those before operation, and there was significant difference between them（P 〈 0.05）. The ALT, AST, ALB and TBIL after operation were same to those before operation, and there was no significant difference between them（P 〉 0.05）. Conclusion The effect of transjugular intrahepatic portosystemic shunt in treating portal hypertension about cirrhosis of live is obvious and it is safe, which is worthy of using.
