低分子量肝素治疗急性脑梗塞的临床研究

观察低分子量肝素（LMWH）对急性脑梗塞的疗效。方法：试用LMWH腹壁皮下注射治疗急性脑梗塞30例，以低分子右旋糖酐静脉滴注治疗24例作为对照。结果：LMWH组与对照组的显效率分别为66．7％与45．8％（P＜0．05），总有效率分别为86．7％与66．7％（P＜0．05），LMWH组功能恢复明显优于对照组（P＜0．05）。LMWH组17例治疗后与治疗前作CT或MRI检查，显示脑梗塞灶体积明显缩小（P＜0．01）。治疗中未出现自发性出血。结论：初步认为LMWH对急性脑梗塞有较肯定的疗效，且方法简便、易行、安全。

Clinical Study on Low Molecular Weight Heparin for Treatment of Acute Cerebral Infarction

Objective:To investigate the therapeutical efficacy of low molecular weight heparin (LMWH) intreating acute cerebral infarction. Methods:LMWH was used to treat 30 patients with acute cerebral infarctionby abdominal wall subcutaneous injection,while 24 matched patients as controls were treated intravenously withdextran-40. Results: The apparent effective rate was 66. 7% in the LMWH group as opposed to 45. 8% in thedextran-40 group(p<0. 05),while the general effective rates were 86. 7 % and 66. 7% in the LMWH and dextran-40 groups respectively(p<0. 05). The degree of neurological function recovery was significantly greater inthe LMWH group than in the dextran-40 group(p<0. 05),and the infarct volumes showed on CT or MRI scandecreased significantly in 17 cases of the LMWH group after treatment with LMWH(P<0. 01). No spontaneoushemorrhage occurred during treatment with LMWH. Conclusion: These findings suggest preliminarily thatLMWH has a definite therapeutical efficacy for the treatment of acute cerebral infarction,and that it's a safe andsimple therapeutical method.
