多巴酚丁胺联合低分子肝素治疗肺心病心力衰竭

目的探讨多巴酚丁胺联合低分子肝素治疗肺心病心力衰竭的疗效及安全性。方法将124例住院肺心病心力衰竭患者随机分为治疗组和对照组,两组患者抗感染、吸氧、改善通气和利尿等基础治疗相同。在此基础上,治疗组:5%GS200mL+多巴酚丁胺10mg静滴,低分子肝素5000U皮下注射,1次/d,疗程10d。对照组:5%GS200mL+利其丁10mg静脉滴注,1次/d,疗程10d。并监测两组患者治疗前后血压、心率、肝肾功能、凝血酶原时间及不良反应。结果治疗组、对照组的总有效率分别为89.0%、86.7%(P>0.05),治疗组、对照组的不良反应发生率分别为3.1%、31.6%(P<0.01)。结论多巴酚丁胺联合低分子肝素治疗肺心病心力衰竭疗效肯定,可大大降低患者不良反应的发生。

Combination of dobutamine with low molecular heparin on cor pulmonale with heart failure

Objective To explore the therapeutic effect and safety of dobutamine with low molecular heparin on cor pulmonale with heart failure.Methods124 patients of cor pulmonale with heart failure in our hospital were divided into the treatment group and control group randomly.The basic treatment was the same in both groups,on the basis,treatment group was treated with 5% GS 200 mL + dobuanmine 10 mg intravenously,and the low molecular heparin 5 000 U subcutaneously,once a day,the course was 10 d.The control group was treated with 5% GS 200 mL + phentolaminum 10 mg intravenously,once a day,the course was 10 d.The blood pressure,heart rate,liver and renal functions,prothrombin time and adverse effects were observed before and after treatment.ResultsThe total effective rate of treatment group and control group were 89.0% and 86.7%(P>0.05),and the incidence of adverse reactions in treatment group and control group were 3.1% and 31.6%(P<0.01).ConclusionThe therapeutic effect of dobutamine with low molecular heparin on cor pulmonale with heart failure is affirmative,and it can reduce the adverse reactions.
