双髂内动脉球囊闭塞术在凶险型前置胎盘剖宫产中的应用价值

目的探讨双髂内动脉球囊闭塞术在凶险型前置胎盘剖宫产中的临床应用价值。方法选择50例凶险型前置胎盘产妇,分为观察组及对照组,各25例,观察组实施剖宫产联合双髂内动脉球囊闭塞术,对照组实施常规剖宫产,比较两组患者术中及术后情况。结果观察组术中出血显著少于对照组（P〈0.05）,手术时间显著短于对照组（P〈0.05）,术后住院时间显著短于对照组（P〈0.05）,观察组需要再次手术、出现凝血功能障碍、失血性休克及下肢静脉栓塞的比率均显著低于对照组（P〈0.05）。结论双髂内动脉球囊闭塞术在凶险型前置胎盘剖宫产中应用,能够减少术中及术后出血,具有较好的临床效果。

The Value of Balloon-assisted Occlusion of Bilateral Internal Iliac Arteries Applied to the Cesarean Section for Dangerous Type Placenta Previa

Objective To explore the clinical value of balloon-assisted occlusion of bilateral internal iliac arteries applied to the cesarean section for dangerous type placenta previa. Methods 50 pregnant women with dangerous type placenta previa were selected and divided into the observation group and the control group with 25 cases in each. The observation group was treated by cesarean section combined with balloon-assisted occlusion of bilateral internal iliac arteries, and the control group was treated by the conventional cesarean section. And the intraoperative and postoperative conditions were compared between the two groups.Results The intraoperative blood loss of the observation group was significantly less than that of the control group（P〈0.05）, operative time was obviously shorter than that of the control group（P〈0.05）, and postoperative hospital stay was significantly shorter than that of the control group（P〈0.05）, the rate of needing second operation, incidence of coagulation disorders, hemorrhagic shock and lower extremity venous embolism in the observation group were significantly lower than those in the control group（P〈0.05）. Conclusion Balloon-assisted occlusion of bilateral internal iliac arteries applied to the cesarean section for dangerous type placenta previa can reduce the intraoperative and postoperative bleeding with good clinical effect.
