全麻复合腰硬联合麻醉用于老年患者髋关节置换术的临床研究

目的 探讨全麻复合腰硬联合麻醉在老年患者髋关节置换术的应用效果。方法 随机抽取该院2012年2月—2013年8月共收治的60例行髋关节置换术老年患者,将其分为观察组与对照组。两组均给予全身麻醉,观察组在全身麻醉基础上加腰硬联合麻醉,对比两组的麻醉效果。结果 观察组的低血压、高血压、术后躁动发生率分别为23.3%、36.7%、36.7%,参照组的分别为63.3%、80.0%、83.3%,观察组的低血压、高血压及术后躁动发生率显著低于对照组;观察组的术后苏醒时间为（15.4±6.2）min,对照组则为（40.3±8.2）min,观察组苏醒时间明显少于对照组;两组比较差异有统计学意义（P〈0.05）;两组的出血量则差异无统计学意义（P〉0.05）。结论 全麻复合腰硬联合麻醉应用于老年患者髋关节置换术,效果显著,值得在临床推广。

Clinical Study of Combined Spinal Epidural Anesthesia Combined with General Anesthesia for Elderly Patients with Hip Joint Replacement

Objective To investigate the application effect of combined spinal epidural anesthesia combined with general anessia on elderly patients with hip replacement. Methods 60 cases of elderly patients underwent hip replacement in our hospital from February 2012 to August 2013 were randomly selected and divided into the observation group and the control group. Both groups were given general anesthesia. In addition, the observation group was given combined spinal epidural anesthesia. The anesthesia effects of two groups were compared. Results In the observation group, the incidence of hypotension, hypertension, and postoperative agitation was 23,3%, 36.7%, 36.7%, respectively, and that of the control group was 63.3%, 80.0%, 83.3%, respectively, the incidence of hypotension, hypertension and postoperative agitation of the observation group was significantly lower than that of the control group, respectively; the postoperative recovery time of the observation group was （15.4±6.2） min, and that of the control group was （40.3 ± 8.2） min, the observation group＇s postoperative recovery time was obviously less than that of the control group, the difference was statistically significant （P〈0.05）; there was no significant difference in the amount of bleeding between the two groups（P〉0.05）. Conclusion Combined spinal epidural anesthesia combined with general anesthesia has significant effect on elderly patients undergoing hip replacement, and it is worthy of clinical promotion.
