超声造影评价肾移植术后延迟恢复的临床研究

摘    要:目的分析肾移植术后延迟恢复的超声造影特征及其应用价值。方法对44例移植术后患者进行超声造影术并进行时间-强度分析,比较移植肾延迟恢复组及移植肾正常恢复组之间的差异。结果与移植肾正常恢复组相比,移植肾延迟恢复组起始时间早、达峰时间短,起始强度及峰值强度高(p<0.05),不同皮质区域的强度参数差异增大、时间参数差异缩小(p<0.05)。结论与同期移植肾正常恢复患者相比,延迟恢复患者移植肾微循环灌注表现为起始时间及达峰时间早且起始强度及峰值强度高的高灌注状态,且在同一患者的移植肾不同皮质区造影剂灌注强度差异较大,而灌注时间差异较小。

CONTRAST ENHANCED ULTRASOUND IMAGING FOR ASSESSING DELAYED GRAFT FUNCTION IN KIDNEY TRANSPLANTATION

Abstract:Objective To analyze the characteristics of contrast - enhanced Ultrasound imaging of delayed graft function in kidney transplantation and the value of its application. Methods 44 cases after kidney transplantation were carried out SonoVue contrast- enhanced ultrasonography (CEUS) and time- intensity curves (TIC) analysis. Compare the difference between delayed graft function (DGF) group and normal graft function (NGF) group. Results Compared with NGF group, the arrival time (AT) of DGF group was earlier, time to peak (TFP) was shorter, and arrival intensity (AI) and peak intensity (PI) were higher (p 〈 0. 05 ) than those in NGF group. In different areas of transplanted kidney cortex of DGF group, the differences of intensity parameters increased, and the differences of the time parameters decreased (p 〈 0. 05 ). Conclusion In comparison with NGF group, microcirculatory per- fusion changes of transplanted kidney in DGF group showed earlier AT and TIP, and higher AI and PI. The differences in different ar- eas of transplanted kidney cortex in AI and PI on the same patient were big, and the differences in AT and TTP were small.
