社会支持对老年肺结核患者应付方式及生活质量的影响

目的探讨社会支持对老年肺结核患者应付方式及生活质量的影响。方法对符合2000年第六次中华医学会全国结核病学术会议修订的肺结核诊断标准的76例老年肺结核患者进行量表测评．以社会支持评分分组．社会支持总分〈35分为低社会支持组。〉45分为高社会支持组，比较两组老年应付问卷（WOCsl、世界卫生组织生存质量测定量表简表（WHOQCL-BREF）评分情况．并进行相关分析。结果76例老年肺结核患者SSRS总分及主观支持、客观支持、支持利用度因子评分分别为：（34．68±6．35）分，（17．08±5．92）分，（12．37±4．31）分，（7．17±2．39）分。其中〈35分者（低分组）28例；〉45分者（高分组）23例；35～45分者25例。社会支持高分组老年肺结核患者生理领域、心理领域、社会关系领域、环境领域评分．显著高于社会支持低分组．差异有显著性（P〈0．01）。结论社会支持程度对老年肺结核患者应付方式及生活质量具有重要影响，良好的社会支持有助于患者采取积极地应付方式、提高患者的生活质量。

The Impact on Coping Style and Life Quality of Social Support on Elderly Patients with Pulmonary Tuberculosis

OBJECTIVE To investigate the impact on coping style and life quality of social support on elderly patients with pulmonary tuberculosis. METHODS Appraising 76 elderly pulmonary tuberculosis patients meeting the diagnostic criteria revised by the sixth National Conference on Tuberculosis of Chinese Medical Association in 2000 with evaluation scale, and grouping by social support score. Total social support score lower than 35 is divided into the low social support group, and higher than 45 is divided into the high social support group. Comparing the score of WOCS and WHOQCL-BREF of two groups, and making correlation analysis. RESULTS 76 elderly pulmonary tuberculosis patients＇ total scores of SSRS, subjective support, objective support and support using degree respectively is （34.68±6.35）, （17.08±5.92） , （12.37±4.31） , （7.17±2.39）. There are 28 cases lower than 35 points （low score group） , 23 cases higher than 45 points （high score group） , and 25 cases between 35 and 45 points. WHOQCL-BREF evaluation of physiological, psychological, social relations and environmental field of the high social support group is significantly higher than the low social support group, and there are significant difference（P〈0.O1 ）. Elderly pulmonary tuberculosis patients＇ social support and factor scores are mostly significant positive correlation （P 〈0.05 or 〈0.01） with confrontation, trivialization, exploration and positive response of WOCS evaluation and WHOQCL-BREF evaluation, yet there are significant negative correlation （P 〈0.05 or 〈0.01） with evaluation of fantasy, escape and negative response. CONCLUSIONS The degree of social support has an important impact on elderly pulmonary tuberculosis patients＇ coping style and life quality, and good social support helps patients take an active approach to deal with and improve the quality of life.
