磨玻璃密度结节肺腺癌MDCT表现

目的：探讨磨玻璃密度结节肺腺癌的多排螺旋CT（ MDCT）的影像表现。方法：分析经病理证实的磨玻璃密度结节（ GGNs）肺腺癌31例的完整MDCT资料，经多方位重建和多窗位处理，对GGNs的形态（圆形或类圆形、不规则形），瘤肺界面（清楚毛糙、模糊无界），边缘形态（短密毛刺、浅分叶、尖角征），内部结构（实性成分、支气管充气征、空泡征），邻近结构改变（胸膜凹陷征、血管集束征）及强化幅值深入分析。结果：瘤体圆形与卵圆形29例、瘤肺界面清楚27例，分叶征19例，毛刺征14例，胸膜凹陷征13例，空泡征11例，血管集束征8例，支气管充气征7例，GGNs内实性成分强化幅值大于20Hu 15例。结论：仔细识别磨玻璃密度结节肺腺癌MDCT征象细节对提高GGNs肺腺癌诊断及其鉴别诊断有较好的应用价值。

MDCT findings of lung adenocarcinoma with glass density

Objective:To investigate the imaging features of multi slice spiral CT( MDCT)in the lung adenocarci-noma of the ground glass density nodule. Methods:To analyze the pathology of ground glass density nodules( GGNs) in adenocarcinoma of lung cancer in 31 cases with complete MDCT data,the MPR and multi window processing,of GGNs morphology( round or oval,irregular shape),lung tumor interface( clearly rough,fuzzy unbounded),edge shape( short dense burr,shallow lobulation,cusp sign),internal structure( solid components and bronchial air sign), and adjacent structures change( pleural indentation sign,blood vessel bundle syndrome)and enhance the perform-ance of the in-depth were analyzed. Results:In 29 cases of tumor body round and ovoid,lung tumor interface clearly was found in 27 cases,leaf syndrome in 19 cases,spiculation in 14 cases,pleural indentation sign in 13 cases,vacuole sign in 11 cases,vascular convergence sign in 8 cases,7 cases of air bronchogram,GGNs solid components enhanced amplitude 40~60Hu in 15 cases. Conclusion:Careful identification of the MDCT features of lung adenocarcinoma with glass density nodule is of good value in improving the diagnosis and differential diagnosis of GGNs lung adenocar-cinoma.
