| 摘 要： | 目的探讨分化型甲状腺癌患者术后血清甲状腺球蛋白（Tg）监测的影响因素。方法对83例分化型甲状腺癌患者的血清抗Tg抗体、TSH和Tg水平进行检测，并分析不同情况下血Tg监测的随访价值。结果在内分泌治疗期间Tg水平〉30μg/L时，停止内分泌治疗后Tg水平才会出现明显升高（P〈0．05）；而当血清中抗Tg抗体〉100 IU／ml时，血Tg检测结果的假阴性率明显增高（P〈0．01）。结论Tg是分化型甲状腺癌较好的随诊指标，在长期监测血Tg水平时不需常规停止内分泌治疗，但需要同时检测患者的血清中抗Tg抗体水平。

| Abstract: | Objective To analyze the influential factors of thyroglobulin（TG） after thyroidectomy in patients with differentiated thyroid carcinoma. Methods Serum Antithyroglobulin antibody, TSH and Tg were monitored after thyroidectomy in 83 patients with differentiated thyroid carcinoma. Results In patients with TG 〉 30 Ixg/L during endocrine treatment,TG rose significantly only after stopping the endocrine treatment （ P 〈 0.05）. The false negative rate of thyroglobulin detection rose significantly in patients with antithyroglobulin antibody 〉 100 IU/ml （ P 〈 0.01 ）. Conclusion Tg might be a good marker in monitoring patients with differentiated thyroid carcinoma after thyroidectomy. Endocrine therapy does not influence the Tg detection, but the level of antithyroglobulin antibody should also be monitored. |
