下颈椎骨折脱位术式的选择

目的探讨三种手术方式治疗下颈椎骨折脱位的适应证及临床疗效。方法回顾性总结2007年4月至2010年1月,分别采用颈椎前路、后路、前后联合入路手术减压、植骨内固定治疗59例下颈椎骨折脱位患者病历资料,其中前路手术组24例,后路手术组8例,一期前后联合手术组27例,分析其临床表现、手术方法及手术前后神经功能恢复、脱位纠正及植骨融合等情况,评估手术疗效。结果三组患者术后脊髓感觉及运动ASIA评分较术前均有不同程度增加,差异有统计学意义（P〈0.05）;术前椎体间移位平均（6.55±2.56）mm,术后为（1.04±0.37）mm,术前脱位椎体间成角（Cobb′s角）平均（16.80±7.02）°,术后为（2.71±1.45）°（P〈0.01）;术后随访6个月～3年,平均18个月,椎体间植骨全部融合。结论采用颈椎前路、后路、前后联合入路手术治疗下颈椎骨折脱位均可使损伤节段获得早期稳定,患者术后神经功能得到一定恢复,根据损伤类型、损伤节段以及脊髓受压部位、程度等综合因素选择手术方式是手术成功的关键。

The Operation Selection of Surgical Approaches in Fracture and Dislocation of Lower Cervical Spine
