摘    要:
【目的】探讨闭合复位石膏夹板固定治疗桡骨远端骨折的疗效。【方法】回顾性分析闭合复位石膏夹板固定治疗100例桡骨远端骨折的临床资料。根据骨折的A0分型分为三组,其中A型35例(A组),B型34例(B组),C型31例(C组),比较三组影像学测量参数及临床评分。【结果】所有患者均获得为期12个月随访,骨折愈合时间为7(6~8)周,在末次随访的臂-肩-手功能障碍评分和掌倾角、尺偏角方面,组间比较差异有显著性(P〈0.05),满意度评分方面,三组间比较A组和B组显著优于C组,A与B组差异无显著性(P〉0.05)。【结论】采用闭合复位石膏夹板固定治疗AO-C型桡骨远端骨折的疗效劣于A或B型患者。要想实现更佳的功能及影像学效果建议手术复位固定治疗。

Abstract:
[Objective]To explore the efficacy of closed reduction and cast splintage for the treatment of distal radius fracture. [Methods] Clinical data of 100 patients with distal radius fracture undergoing the closed reduction and cast splintage were analyzed retrospectively. According to AO classification o{ fracture, all patients were divided into 3 groups including group A(type A, n =35), group B(type B, n =34) and group C (type C, n = 31). Radiographic measurement parameters and clinical scores were compared among three groups. ['Results] All patients were clinically and radiographically followed up for 12 months. Fracture healing time was 7 weeks(6-8 weeks). There were significant differences in arm-shoulder-hand score, volar tilt angle and ulnar drift angle at final follow up among groups( P〈0.05 ), but there was no significant difference in satisfaction score among groups( P 〉0.05). [Conclusion] The efficacy of closed reduction and cast splintage for the treatment of AO-C distal radius fracture is inferior to type A or B patients. Operative reduction and fixation is recommended to achieve better functional and radiographic results.
