内镜下氩离子凝固术与注射术治疗非食管静脉曲张性上消化道出血疗效分析

目的探讨内镜下氩离子凝固术（argon plasma coagulation，APC）治疗上消化道出血的临床应用价值。方法采用氩气刀对非食管静脉曲张性上消化道出血患者进行治疗（APC组），以内镜下注射肾上腺素止血为对照组（注射组），比较两组的有效止血率。结果注射组的患者中，112例患者未再出血。4例患者再出血，总有效率96．6％。APC组的患者中，119例患者未再出血，2例患者再出血，总有效率98．3％。结论APC组及注射组止血有效率高，无明显差异性（P〉0．05）。但内镜下氩离子凝固术治疗非食管静脉曲张性上消化道出血具有疗效确切，安全快速，并发症少，患者痛苦小等特点，临床应用价值较高。

Curative effect analysis on the endoscopic APC in treating non-variceal upper gastrointestinal hemorrhage

Objective To explore the clinical application of endoscopic APC(argon plasma coagulation)in the treatment of patients with upper gastrointestinal hemorrhage. Method The hemostatic effectiveness of APC and injection of epinephrine was compared. Result The injection group was treated by adrenaline injection in 116 patients with upper gastrointestinal hemorrhage, hemorrhage in 114 patients ceased and 4 patients had rehemorrhage. The rate of hemostatic effectiveness was 96.6%. Patients in APC group was treated by APC in 121 patients with upper gastrointestinal hemorrhage, hemorrhage in 119 patients ceased and 2 patients had rehemorrhage. The rate of hemostatic effectiveness was 98.3%. There was no significant difference between two groups as for hemostatic effectiveness (P > 0.05). Conclusion Endoscopic argon plasma coagulation for the treatment of patients with upper gastrointestinal hemorrhage is safe, and effective, painless.
