卡维地洛联合依那普利治疗风湿性心脏病慢性心力衰竭的疗效分析

目的 观察卡维地洛联合依那普利治疗风湿性心脏病慢性心力衰竭的临床疗效及对患者心脏重构的影响.方法 将我院收治的风湿性心脏病慢性心力衰竭患者72例,随机均分为观察组36例和对照组36例,对照组患者给予常规的强心、扩血管、利尿等治疗,观察组患者在常规治疗的基础上加用卡维地洛和依那普利治疗,治疗6个月后,应用彩超机测定心脏结构相关指标,观察并对比两组的临床疗效.结果 经过6个月治疗,观察组左房直径(LAD)、左室舒张末期内径(LVEDD) 、左室收缩末期内径(LVESD) 显著下降,左室射血分数(LVEF) 显著增加,与对照组比较有显著差异(均P<0.05);观察组有效率为90.9%,明显高于对照组有效率(63.6%),差异具有统计学意义(P<0.05).结论 在应用常规治疗的基础上,加用卡维地洛联合依那普利治疗风湿性心脏病慢性心力衰竭,能够明显改善患者心功能,抑制心脏重构,提高临床疗效,值得临床推广应用.

Clinical effect of carvedilol and enalapril on chronic heart failure caused by rheumatic heart disease

Objective To analyze the effect of carvedilol and enalapril on chronic heart failure(CHF) caused by rheumatic heart disease (RHD).Methods Seventy two RHD patients with CHF were randomly divided into observation group (n=36) and control group (n=36).The control group was treated with a conventional regimen,while the observation group was treated with carvedilol and enalapril on basement of the conventional regimen.Results Left ventricular end-diastolic diameter (LVEDD) and left ventricular end-systolic diameter (LVESD) in the control group were significantly lower than that of the control group. Left ventricular ejection fraction in the observation group was much higher (P<0.05); The effective rate of the observation group (90.9%)was higher than that of the control group(63.6%) with statistical significance(P<0.05).Conclusion Combination therapy of carvedilol and enalapril is effective on patients with CHF caused by RHD,which can inhibits ventricular remodeling.
