影响颅内动脉瘤手术预后的相关因素分析

目的 探讨影响颅内动脉瘤手术预后的因素。方法 回顾性分析1 041例开颅治疗的动脉瘤患者的临床资料。对可能的影响因素和手术结果进行单因素和多因素分析。结果患者人院Hunt和Hess分级为3或4级、后循环动脉瘤、年龄>60岁、术中发生动脉瘤破裂、动脉瘤直径>15mm、术前不发生蛛网膜下腔出血者,均提示为手术后预后不良的因素。结论 动脉瘤的手术预后受多种因素影响,明确影响动脉瘤手术预后的因素并采取相应的治疗措施,有助于进一步提高动脉瘤的手术治疗效果。

Factors affecting the prognosis of intracranial aneurysm operation

Objective To study the factors affecting the prognosis of intracranial aneurysm operation. Methods A series of 1 041 patients with intracranial aneurysm between 1990 to 2002 who were treated surgically in our department were analyzed retrospectively. Factors that might affect the result of operation were analyzed by single-factor and multi-factor. Results Factors that have a significant association with poor prognosis were Hunt & Hess 3-4 grade,back circulation aneurysm,older than 60,aneuryzm ruptured during the operation,aneuryzm larger than 15mm in diameter and free of subarachnoid hemmorrhage. Conclusions The prognosis of operation is affected by many factors. Confirmation of these factors and taking appropriate measures to cope with them is helpful for better operation results.
