妊娠高血压性心衰36例临床分析

目的：分析妊娠高血压性心衰的临床特征，探讨其诱发因素及预防措施。方法：回顾性分析我院2000年1月至2007年12月收治的36例妊娠高血压心衰的患者的临床资料。结果：并发低蛋白血症30例，产前贫血13例，严重水肿20例，使用硫酸镁时发生心衰2例，双胎妊娠4例，扩容不当6例，并肺炎2例。36例均痊愈出院。结论：妊娠期高血压疾病本身或合并贫血、低蛋白血症、严重水肿、多胎妊娠和呼吸道感染是妊娠期高血压性心衰发病的诱发因素。积极治疗原发病，防治贫血及低蛋白血症，掌握早期心衰的诊断和扩容指征，慎用硫酸镁，可减少妊娠高血压性心衰。

Clinical Analysis of Heart Failure caused by Pregnancy-induced Hypertension

Objective: To explore the clinical feature and susceptible factors and preventive measure of heart failure caused by pregnancy-induced hypertension. Methods: The clinical data of 36 pregnancy women with heart failure caused by pregnancy-induced hypertension in our hospital from Jan,2000 to Dec,2007 were analyzed retrospectively. Results: The number of patients complicated with hypoproteinemia, general edema, using magnesium sulfate, twin pregnancy, improper dilatancy, pneumonia were 30,13,20,2,4,6 and 2 respectively. All of the patients were cured. Conclusion:Pregnancy-induced hypertension itself and complicating with anemia, hypoproteinemia, general edema, multiple pregnancy and respiratory tract infection are the susceptible factors for heart failure. The following measure will help us to prevent heart failure caused by pregnancy-induced hypertension:intensive treatment for pregnancy-induced hypertension, prevention and correction for hypoproteinemia and anemia,mastering the diagnosis for early heart failure and the indication for dilatancy, using magnesium sulfate cautiously in high risk.
