类风湿关节炎患者类风湿因子与红细胞沉降率的相关性

目的 探究类风湿关节炎(RA)患者类风湿因子(RF)与红细胞沉降率(ESR)的关联性,为临床诊断指标提供参考.方法 收集类风湿关节炎患者77例,男性22例,女性55例,采用速率散射比浊法检测血清类风湿因子,魏氏法测定红细胞沉降率,采用SPSS13.0统计分析血沉与类风湿因子的阳性率及相关性.结果 男性组ESR中位数为48mm/h,95%可信区间为22.00-133.95mm/h;RF中位数为32.00IU/ml,95%可信区间为10.10-458.10IU/ml,ESR与RF阳性率无统计学差异(χ2=1.414,P=0.157),存在显著正相关(r=0.549,P=0.032).女性组ESR中位数为40mm/h,95%可信区间为12.00-134.65mm/h;RF中位数为40.10IU/ml,95% 可信区间为10.10-570.10IU/ml,ESR与RF阳性率无统计学差异(χ2=1.291,P=0.197),存在显著正相关(r=0.651,P=0.011).结论 RA的ESR、RF的阳性率存在显著的性别差异.RA患者的RF与ESR呈正相关关系,不存在性别差异性;对于ESR阳性但RF阴性或RF阳性但ESR阴性的患者需进一步检查.

Correlation between rheumatoid factor and erythrocyte sedimentation rate in patients with rheumatoid arthritis

Objective To provide the reference for the diagnosis and treatment of rheumatoid arthritis(RA) by studying the correlation between rheumatoid factor(RF) and erythrocyte sedimentation rate(ESR) in RA patients.Methods Seventy-seven RA patients(22 males and 55 females) were enrolled in this study.Their RF and ESR were detected by nephelometry and Wilcoxon method,respectively.Positive rate of ESR and RF and their correlation were analyzed with SPSS 13.0.Results The median ESR and 95% CI were 48mm/h and 22.00-133.95mm/h respectively while the median RF and 95% CI were 32.00IU/ml and 10.10-458.10 IU/ml respectively in male patients.No significant difference was found in the positive rate of ESR and RF(χ2=1.414,P=0.157).However,ESR was positively correlated with RF(r=0.549,P=0.032).The median ESR and 95% CI were 40mm/h and 12.00-134.65mm/h respectively while the median RF and 95% CI were 40.10IU/ml and 10.10-570.10IU/ml respectively in female patients.No significant difference was found in the positive rate of ESR and RF(χ2=1.291,P=0.197).However,ESR was positively correlated with RF(γ=0.651,P=0.011).Conclusion The positive rate of ESR and RF is significantly different in male and female RA patients.RF is positively correlated with ESR in RA patients.RA patients with positive ESR but negative RF,or with positive RF but negative ESR should be further examined.
