CT引导下经下颌切迹鼻咽部穿刺活检33例

目的]评价CT引导下经下颌切迹鼻咽部穿刺活检术的临床应用价值.方法]经鼻咽横断位CT扫描确定病变层面后,参照患侧下颌切迹至鼻咽部病灶中心点连线的角度及深度,用18G切割活检枪行病灶组织学活检.结果]33例均获得病理学诊断,其中鳞癌22例,未分化癌5例,泡状核细胞癌2例,慢性炎症4例.结论]经皮下颌切迹至鼻咽部的解剖学途径避开了重要及粗大的神经血管结构,经该路径行活检术是安全可行的.CT引导下鼻咽部穿刺活检是鼻咽镜下咬检结果阴性的病例取得病理结果的一种有效方法.

Clinical Application of CT-guided Nasopharyngeal Needle Biopsy Through Mandibular Notch in 33 Cases

Purpose] To evaluate the value of clinical application of CT-guided nasopharyngeal needle biopsy through mandibular notch. Method] The section of the lesion was identified after transsectional scanning of nasopharynx. The lesion was punctured with 18G dissection biopsy gun after measuring the angle and depth between the mandibular notch and focus of lesion. Results] All of 33 cases acquired pathological results,among them 22 cases were squamous cell carcinoma; 5 cases, undifferentiated carcinoma; 2 cases, vesicular nucleus cell carcinoma; and 4 cases, chronic inflammation. Conclusion] The CT-guided nasopharyngeal needle biopsy through the mandibular notch is feasible and safe. Great blood vessel and nerves injuries are avoided. It is suitable for epipharyngoscope routine biopsy negative cases.
