颈部蛛网膜下腔注射血肿液化剂治疗稳定期脑出血的效果观察

目的观察颈部蛛网膜下腔注射血肿液化剂治疗稳定期脑出血的临床效果。方法将本院脑病科经保守治疗稳定期脑出血患者按治疗方法分为治疗组366例与对照组298例。治疗组应用蛛网膜下腔注射血肿液化剂（尿激酶10000，30000U＋透明质酸酶1500U）治疗，对照组应用10％葡萄糖250ml＋脑蛋白水解物注射液10ml＋胞二磷胆碱0．75g静脉滴注治疗。结果治疗组与对照组的基本痊愈率分别为44_3％和27．5％，两组比较，差异有统计学意义（P〈O．05）；治疗组出血量≤30ml、出血量〉30ml的基本痊愈率分别为49．6％、33．3％，对照组分别为32．6％、17．6％，两组比较，差异有统计学意义（P〈O．05）；两组治疗1个月后均经头颅CT扫描，显示治疗组血液全部吸收234例，占63．9％，对照组298例，占35．2％，两组比较，差异有统计学意义（P〈O．01）。结论颈部蛛网膜下腔注射血肿液化剂治疗稳定期脑出血的效果较对照组显著，且对脑内血液的吸收较对照组快而完全。

Effect observation of cervical subarachnoid injection of hematoma liquefi- er treating cerebral hemorrhage of stable period

Objective To observe the clinical efficacy of cervical subarachnoid injection of hematoma liquefier treating cerebral hemorrhage of stable period. Methods The patients with cerebral hemorrhage of stable period treated by con- servative treatment in our department were randomly divided into the treatment group （n=366） and the control group （n=296） according to the treatment methods.The treatment was treated by subarachnoid injection of hematoma liquefier （10 000-30 000 U urokinase and 1500 U hyaluronidase） while the control group was treated by 250 ml 10% glucose, cerebroprotein hydrolysate injection of 10 ml and cytoplasmic two choline of 0.75 g for intravenous drip. Results The basic recovery rate of the treatment group and the control group were 44.3% and 27.5% respectively and there was a statistical difference between the two groups （P〈O.05）;the basic recovery rate for amount of bleeding ~〈30 ml and amount of bleeding〉30 ml in the treatment group were 49.6%,33.3% while the rate was 32.6%,17.6% in the control group respectively and there was a statistical difference between the two groups （P〈O.05）;After one month treatment,the two groups were given cranial CT scan and it showed that there were 234 cases （63.9%）with the whole blood absorption in the treatment group while that of the control group was 105 cases （35.2%） and there was a statistical difference be- tween the two groups （P〈0.01）. Conclusion Compared with the control group,the effect of cervical subarachnoid injec- tion of hematoma liquefier treating cerebral hemorrhage of stable period is obvious,and the brain blood absorption is faster and more complete.
