下颌髁状突骨折87例治疗及疗效评价

目的评价髁状突骨折保守治疗与手术治疗的效果，为临床在治疗中选择合适的方式提供参考。方法回顾分析了我院87例髁状突骨折的临床资料，随访检查患者的面形、张口度、张口形、咬牙合]关系、关节症状，并摄X线片或CT检查骨折愈合情况。结果髁状突骨折的保守治疗和手术治疗总体疗效都比较满意，对比2组之间无明显差异。手术摘除髁状突4例疗效不满意，平均张口度23mm，咬牙合]关系出现紊乱。结论保守治疗仍可作为大多数髁状突骨折治疗的首选方法，手术治疗应严格选择适应证，摘除髁状突应尽量少用。

The treatment and effects of fractures of mandibular condylar:an analysis of 87 cases

Objective The purpose of this study was to evaluate the effects of condylar process fractures treated by conservative or surgical methods.Methods The clinical records of 87 patients with the fracture of mandibular condyle treated at Weinan Central Hospital between 1987 and 2005 were retrospectively reviewed.The general clinical information and treatment data were analyzed.Results No statistical differeneces between conservatively and surgically treated fractures were discovered,but 4 cases removed condyle were unsatisfactory in effects. Conclusion The conservative treatment is first for many mandibular condylar fractures.Indication for surgical reduction should be controlled strictly and removal of condyle is adopted as less as possible.
