荧光定量聚合酶链反应检测细胞角蛋白19在乳腺癌诊疗中的应用

摘    要:
目的 探讨荧光定量聚合酶链反应 (FQ PCR)技术检测细胞角蛋白 19(CK19)基因表达在乳腺癌诊疗中的应用价值。方法 采用FQ PCR法 ,以甘油醛 3 磷酸脱氢酶 (GAPDH)为内对照 ,测定 15名健康女性体检者、2 5例良性乳腺疾病和 76例乳腺癌患者外周血中CK19的表达量。结果 CK19和CK19/GAPDH在正常对照组和良性乳腺疾病组间的差异无显著意义 (P >0 0 5 ) ,而乳腺癌组则显著高于前两组 (P <0 0 5 ) ,GAPDH在三组间的差异均无显著意义 (P >0 0 5 )。乳腺癌组的CK19和CK19/GAPDH阳性率为 5 3 9% (41/ 76 ) ,而另两组均为阴性。 2 7例乳腺癌患者术前 ,化疗后 1、2 8和5 6dCK19和CK19/GAPDH均呈下降趋势 ,但化疗后 1和 2 8d与术前比较 ,差异无显著意义 (P >0 0 5 ) ,化疗后 5 6d则显著低于术前 (P <0 0 5 )。结论 FQ PCR技术是较灵敏和特异的快速定量检测CK19方法 ,可用于乳腺癌的诊断和疗效监测。

Abstract:
Objective To evaluate the clinical application of fluorescent quantification polymerase chain reaction (FQ-PCR) to detect cytokeratin 19(CK19) in the diagnosis of breast cancer MethodsCK19 in the of clinical samples of 15 health women, 25 patients with benign breast disease and 76 patients with breast cancer were detected by FQ-PCR GAPDH was used as internal controlResults There were no significant difference in CK19 and CK19/GAPDH between normal controls and benign breast disease group( P >005) CK19 and CK19/GAPDH in breast cancer group were higher than those in the other groups( P <005) There were no difference in GAPDH among three groups( P >005)CK19 and CK19/GAPDH in 27 breast cancer patients were declining after chemotherapy on 1 th day,28 th day,56 th day There were no difference among pre-operation and 1 th day and 28 th day( P >005) There was significant difference between pre-operation and 56 th day( P <005)The latter was lower than the former Conclusions FQ-PCR is a rapid, sensitive and specific method for quantitating CK19 It also gives objective evidence to the diagnosis, therapy and metastasis of breast cancer
