摘    要:目的:研究乌司他丁对心肺复苏后早期脑水肿和脑超微结构的变化的影响.方法:建立大鼠心肺复苏模型,120只大鼠随机分成对照组、复苏组和药物组(每组再按时间分即刻、1/2 h、3 h、6 h、9 h五亚组),检测心肺复苏后各时相脑组织水含量及伊文氏兰(EB)含量的变化,以电镜观察各时相脑组织超微结构.结果:复苏组心肺复苏后1/2 h、3 h、6 h、9 h脑组织水含量持续增加,6 h后EB含量升高,电镜观察各时间点见细胞水肿,6 h后V-R间隙明显扩大;药物组仅于复苏后1/2、3 h有脑组织水含量的增加,6 h后水含量回降,各时间点EB含量无明显升高,复苏后6 h开始超微结构改变明显减少.结论:乌司他丁可通过抑制脑水肿,对大鼠心肺复苏后早期脑损伤起到一定的保护作用.

Abstract:Objective:To study the effect of ulinastatin on changes of cerebral edema and the ultrastructure of brain at different time points after early cardiopulmonary resuscitation(CPR) in rats.Methods: The animal model of cardiac arrest(CA) was established.One handred and twenty male Sprague Dawley(SD) rats were randomly divided into 15 groups(n=8,in each) : control group and CPR group and drug group,which were further divided into 5 subgroups respectively on the base of time.The contents of Evens blue(EB) and brain water in cerebral tissue were assayed,and the ultrastructure of brain was observed.Results: The brain edema was obvious at 1/2 h after CPR in CPR group(P<0.05),and was still elevated at 9 h(P<0.05).EB content was higher at 6 h and 9 h than that in normal control group and control group(P<0.05).In animals treated with ulinastatin,brain water content was increased(P<0.05) at 1/2 h to 3 h after CPR,and was comeback after 6 h.EB content didn't change significantly.The observation of the ultrastructure in brain tissue showed that the injury was relieved obviously in the ulinastatin group than that in the CPR group.Conclusion: CPR results primarily in cytotoxic edema,subsequently with the increased permeability of blood-brain barrier,vasogenic edema plays a critical role in formation of brain edema.Ulinastatin can alleviate the cerebral edema and protect from cerebral damnification after early CPR in rats.
