PDCA循环在预防剖宫产切口感染中的应用

目的 应用计划-实施-检查-处理(PDCA)循环对剖宫产实施全面质量管理,预防剖宫产手术切口感染的发生.方法 采用PDCA不断循环的质量管理方法,制定监测计划与方案,制定出标准的操作规程,不断解决遗留问题.结果 2009年12月实施PDCA管理模式前,剖宫产产妇感染率为8.00％,明显高于实施后(2010年1月)的0.97％,差异有统计学意义(x2=4.374,P＜0.05)；2009年12月-2011年11月两年的感染率明显下降,切口感染率2010年为0.92％、2011年为0.33％,监测验证,重视易感因素,医师准确、标准的切开、分离技术及缝合时恢复正常组织结构至关重要.结论 遵循PDCA循环的管理模式,实施全面质量持续不断改进的方法,预防剖宫产切口感染效果明显.

Application of PDCA in prevention of incision infections due to cesarean section

OBJECTIVE To use PDCA cycle for carrying out overall quality management of cesarean section,so as to prevent the surgical incision infections.METHODS By using PDCA quality management method with continuous circulation to establish monitoring plan and program,the standard operation procedure was set up to solve the problems left constantly.RESULTS The incidence of the cesarean section incision infections was 8.00% before the implementation of PDCA(Dec 2009),which was significantly higher than 0.97% after the implementation(Jan 2010),the difference was statistically significant(χ2= 4.374,P<0.05);the infection rate from Dec 2009 to Nov 2011 was significantly decreased,the incidence of the incision infections was 0.92% in 2010,0.33% in 2011,the monitoring result indicated that the emphasis on the predisposing factors,the accurate and standard incision,separation techniques,and restoring normal structure of tissue are very important.CONCLUSION The effect of preventing cesarean section incision infections is obvious by following the management mode of PDCA cycle and carrying out comprehensive continuous quality improvement.
