氢质子磁共振波谱定量分析在脑梗死中的应用

摘    要:
磁共振波谱是基于磁共振成像基本原理、化学位移及自旋耦合现象的一种影像学检查技术,它的应用使无创性的动态观察脑梗死细胞代谢的变化成为可能。脑梗死后代谢物的改变以N-乙酰天冬氨酸和乳酸的变化最为重要,N-乙酰天冬氨酸的明显下降和乳酸的升高是诊断急性脑梗死敏感而有效的指标;梗死灶边缘、周围N-乙酰天冬氨酸正常或轻度下降而乳酸升高的区域提示缺血半暗带的存在;二者的变化亦能对患者的预后做出评估。文章的最后部分对磁共振波谱定量方法问题做了评述。

Abstract:
Magnetic resonance spectroscopy (MRS) is one imaging technique which bases on basic principle of magnetic resonance imaging,chemical shift and spin coupling.The application of MRS has made non-invasive and dynamic investigations of changes of the intracellular metabolites possible in the study of brain infarction.The changes of N-acetylaspartate (NAA) and lactate are most important after infarction,and obvious reduction of NAA and elevation of lactate are sensitive and effective indexes to diagnose acute brain infarction.Marginal and peripheral regions of infarction lesion with normal or slight reduction of NAA and elevated lactate may suggest the ischemic penumbra.The changes of NAA and lactate are valuable for prediction of outcome.The quantitative methods of MRS were discussed in the last part of this article.
