Moss Miami与RF系统治疗胸腰椎骨折的疗效对比

摘    要:目的评价Moss Miami椎弓根螺钉内固定系统治疗胸腰椎骨折的临床效果,并与传统的RF系统进行对比。方法对72例胸腰椎骨折的患者进行回顾性分析,其中35例应用Moss Miami椎弓根钉减压复位内固定,37例采用RF钉减压复位内固定,平均随访19个月。对所有胸腰椎骨折患者术前、术后和随诊时的临床表现、椎体高度及成角畸形等进行比较。结果Moss Miami组患者平均伤椎椎体高度比和Cobb角术前分别是0.49、27.6°,术后分别是0.96、3.4°,随访时分别是0.88、7.2°。而RF组伤椎椎体高度比和Cobb角术前为0.54、22.5°,术后0.91、9.5°,随访时0.75、15.5°。随访时椎体高度有不同程度的丧失,RF组比Moss Miami组丢失显著。结论Moss Miami椎弓根钉内固定系统是治疗胸腰椎骨折的有效方法,与传统RF系统相比具有一定的优势。

Abstract:Objective To assess the clinical result of surgical treatment of thracolumbar fracture by Moss Miami transpedicular system compared with traditional RF instrumentation. Methods This study included 72 Patients with thoracolumbar fracture who received posterior reduction and internal fixafion. Thirty-five patients were operated by Moss Miami transpedicular system, and 37 patients used RF instrumentation. All data of the patients including clinical presentations, radiograph, and CT scan were collected preoperatively, postoperatively and follow-up. The average follow-up was 19 months. Result Before operation, average height loss rate of the injured vertebrae of the Moss Miami group was 0.49, and the Cobb's angle was 27.6°. The average height loss rate of the injured vertebrae and the Cobb's angle were 0.96 and 3.4°, which were 0.88 and 7.2° in the follow-up. Before operation, average height loss rate of the injured vertebrae of the RF group was 0.54, and the Cobb's angle was 22.5°. The average height loss rate of the injured vertebrae and the Cobb's angle were 0.91,9.5°, which were 0.75 and 15.5° in the follow-up.In the follow-up, there were more correction loss in the patients used RF compared with Moss Miami system. Conclusion Moss Miami tranapedicular system is an effective method in the treatment of thoracolumbar fracture. With a better result than traditional RF instrumentation.
