摘    要:目的探讨闭合复位旋股骨近端髓内钉(PFNA)治疗股骨转子间骨折的疗效。方法采用闭合复位PFNA治疗股骨转子间骨折58例,记录手术时间,术中出血量,观察骨折愈合情况。采用Harris评分评估髋关节功能情况。结果手术时间为45~88(55.2±8.9)min,术中出血量80~180(95.5±14.3)ml。54例获随访,时间6~24个月,骨折均骨性愈合;4例失访。Harris功能评分:优13例,良32例,中9例,优良率83.3%。结论闭合复位PFNA治疗股骨转子间骨折,创伤小,操作简便,出血少,固定牢固,并发症少,疗效满意。

Abstract:Objective To study the effect of close reduction and proximal femoral nail anti-rotation(PFNA) in treating femoral intertrochanteric fractures.Methods 58 cases of intertrochanteric fractures were treated with closed reduction and PFNA fixation.Operative time,volume of blood loss were analyzed,and the condition of fracture union was observed and the hip function was evaluated using Harris criterion.Results Operative time ranged 45~88 min with an average time of 55.2±8.9 min,and the volume of blood loss ranged 80~180 ml with an average volume of 95.5±14.3 ml.No intra-operative femoral fractures,no regional or deep infection occurred during hospitalization period.54 cases were followed up for 6~24 months.According to the Harris criterion for evaluation,the results were excellent in 13 cases,good in 32 and fair in 9,and the excellent and good rate was 83.3%.Conclusions Close reduction and PFNA for femoral intertrochanteric fractures has the advantages of minimal invasion,easy anipulation,less blood loss,stable fixation,less complications and satisfactory effect.
