早晚使用普贝生促宫颈成熟效果的观察

摘    要:目的:探讨不同时间使用普贝生促宫颈成熟效6果的差异。方法:采用对照观察的方法,对85例足月单胎的孕妇阴道内使用普贝生栓剂。将其随机分为两组,分别于早上8时至10时(41例,其中2例因胎膜早破剔除)或晚上8时至10时(44例)置药,对第一次用药失败者于24小时后再次阴道置药,且保证两次用药时间基本一致。比较两组孕妇的宫颈Bishop评分、分娩情况以及对胎儿和新生儿的影响。结果:无论放置第一枚还是第二枚普贝生后12小时Bishop评分早晚之间比较有显著性差异,晚上放置者评分高于早上放置者。使用第一枚普贝生时,晚上放药至临产时间短于白天,而使用第二枚普贝生后无差异。无论早上还是晚上,使用第二枚普贝生至临产的时间均短于第一枚。使用第二枚普贝生时,促宫颈成熟率晚上比早上高。剖宫产率、新生儿Apgar评分、临产至分娩时间等方面两组无显著性差异。结论:普贝生是一种促宫颈成熟的有效药物,晚上使用效果优于早上。

Comparison of the effectiveness of prepidil in evening and morning maturation of cervixFt
Abstract:Objective To observe the difference of using pepidil on maturing cervix between evening and morning .Methods 85 women participated in an open randomised study. 41of them (2 were get rejected because of premature rupture of membrane ) received Prepidil intracervically in the morning (8.00a.m.-10.00a.m.) and 44 at night (8.00p.m.-10.00 p.m.). If failed a second prepidil would be given to them at almost the sametime.We observe Bishop score of cervix ,delivery matters and neonatal’S Apgar score. Result The success rate was 79.5% (35 of 44) in the treatment group of the night compared with 71.8% (28 of 39) in the group of morning group.Bishop score are higher in the night group no matter first or second prepidil given . The induction time for contractions in night group was shorter than morning group when using the first prepidil. The induction time for contractions was shorter when using prepidil again no matter in night group or morning group.And the successful rate was higher in night group when using the second prepidil. There were no difference in Cesearean rate ,Apgar score and time for delivery between these group. Conclusion Prepidil is a kind of efective and safe drug for cervix maturing and patrurifacien,and it is better be used at night.
