鼻内镜手术治疗慢性鼻窦炎鼻息肉临床经验总结

目的总结鼻内镜手术治疗慢性鼻窦炎鼻息肉的临床经验.方法对512例慢性鼻窦炎鼻息肉患者分成初期组和近期组进行鼻内镜手术,术后随访1年以上进行比较.结果参照1997海口会议标准,初期组治愈率为61.6%,好转率为33.0%,无效率为5.4%;近期组治愈率为88.4%,好转率为11.6%.结论加强围手术期处理,熟悉鼻窦解剖,术中行中鼻甲成形术,完整切除钩突,同期矫正鼻中隔偏曲,控制术中出血,可有效防止手术并发症发生和提高疗效.

Clinical experience in the treatment of chronic sinusitis and nasal polyps with endoscopic surgery

Objective To sum up the clinical experience in the treatment of chronic sinusitis and nasal polyps with endoscopic surgery (ESS). Methods 512 cases with chronic sinusitis and/or nasal polyps were included in this study. They were divided into two groups ,i. e. the initial group and the group with a little longer delayed ill course. All the cases were treated with ESS and followed up for one year following the therapy. To the end of following up period, the therapeutic effects were compared between the two groups. Results The curative,effective and ineffective rates were 61. 6%, 33. 0% and 5. 4% for the cases in the first group and they were 88. 4%, 11. 6% and O for those in the second group respectively. Conclusions Several factors can give influences on the therapeutic effects of ESS. To obtain the best therapeutic effects from ESS,it is very important to do the perioperation management well and to operate in a best combining way with conchoplasty in the middle turbinate, whole nasal septum correction and effective bleeding-stopping procedures during the operation.
