定量组织速度成像技术在冠心病诊断中的临床价值

目的研究定量组织速度成像（QTVI）技术在冠心病诊断中的临床价值。方法冠状动脉造影（CAG）诊断为冠心病（CAD）患者38例为CAD组;正常体检者30例作为正常对照组。在QTVI模式下取心尖四腔观、心尖两腔观和心尖长轴观。获取CAD组及正常对照组左心室各壁中段共6个心肌节段QTVI的速度-时间曲线图,测量速度指标收缩期峰值速度（Vs）、舒张早期峰值速度（Ve）及舒张晚期峰值速度（Va）,时间指标等容收缩时间（ICT）、等容舒张时间（IRT）、S峰达峰时间（Ts）及E峰达峰时间（Te）。结果 CAD组CAG所示冠状动脉狭窄对应各壁中段Vs、Ve较正常对照组均明显降低（P〈0.05或〈0.01）。CAD组冠状动脉狭窄对应室壁中段的IRT、ICT、Ts较正常对照组均明显延长（P〈0.05或〈0.01）。左心室缺血或梗死节段Vs、Ve与冠状动脉的狭窄程度均呈负相关（P〈0.05或〈0.01）。左心室缺血或梗死节段ICT、IRT与冠状动脉狭窄程度均呈正相关（P〈0.05或〈0.01）。结论应用无创且实时的QTVI技术,通过左心室局部心肌运动速度及时间指标推测其供血冠状动脉是否狭窄,为需要进一步行CAG的患者起到了初筛作用,提高了CAG的阳性率。
