维生素K2及Gas6与维持性血透患者血管钙化相关性研究

摘    要:
目的 探讨维持性血液透析(MHD)患者血管钙化与维生素K2及生长停滞特异性蛋白6(Gas6)的关系.方法 选择我院2013年1月至2016年9月住院的MHD患者60例作为MHD组,并选择同期行体检的健康人群60例作为对照组,比较两组受检者维生素K2和Gas6的表达水平,并分析其与血管钙化评分的关系.结果 MHD组患者血液中维生素K2水平较对照组降低[(2.46±0.93)nmol/L vs(4.17±1.04)nmol/L],而Gas6水平较对照组增高[(42.36±2.23)ng/mL vs(18.86±2.04)ng/mL],差异均有统计学意义(P<0.01);MHD组患者中,血管钙化评分≥4分组患者的维生素K2水平为(1.99±0.77)nmol/L,明显低于评分<4分组的(2.56±0.93)nmol/L,差异有显著统计学意义(P<0.05);血管钙化评分≥4分组患者Gas6水平为(46.82±2.93)ng/mL,明显高于评分<4分组的(38.24±2.77)ng/mL,差异有显著统计学意义(P<0.01).维生素K2及Gas6水平与血管钙化具有明显相关性(γ=-0.276,0.64,P<0.05).结论 维生素K2与Gas6在血透患者中呈现差异性表达,其变化一定程度上反映出血管钙化的严重程度.

Abstract:
Objective To study the relationship between vitamin K2, growth arrest-specific protein 6 (Gas6) and vascular calcification in maintenance hemodialysis patients. Methods From January 2013 to September 2016, 60 patients on hemodialysis (MHD group) and 60 health people (control group) were recruited from Liuzhou Worker's Hos-pital. The vitamin K2 and Gas6 levels were statistically analyzed, and the association between vitamin K2, Gas6 and vas-cular calcification were analyzed. Results The level of vitamin K2 in MHD group was significantly lower than that in the control group with (2.46±0.93) nmol/L vs (4.17±1.04) nmol/L, while the level of Gas6 in MHD group was signifi-cantly higher than that in the control group with (42.36±2.23) ng/mL vs (18.86±2.04) ng/mL, P<0.01. Vitamin K2 level of vascular calcification≥4 points group in maintenance hemodialysis patients was significantly lower than that of<4 points group, (1.99±0.77) nmol/L vs (2.56±0.93) nmol/L, and Gas6 level of≥4 points group was significantly high-er than that of<4 points group, (46.82±2.93) ng/mL vs (38.24±2.77) ng/mL, P<0.01. Spearman analysis demonstrat-ed that there were potential association between vitamin K2/Gas6 and vascular calcification (γ=-0.276, 0.64, P<0.05). Conclusion There is differential expression of vitamin K2 and Gas6 in patients on maintenance hemodialysis, which to some extent indicates the degree of vascular calcification.
