葡萄胎中E-cadherin和Laminin的表达及临床意义

摘    要:目的 探讨E-cadherin和Laminin对部分性葡萄胎、完全性葡萄胎和侵蚀性葡萄胎的诊断价值.方法 收集2005年1月至2016年3月新疆医科大学第一附属医院妇科75例患者的葡萄胎组织,石蜡包埋组织,采用免疫组化SP法检测包埋组织中E-cadherin和Laminin的表达水平,其中部分性葡萄胎25例,完全性葡萄胎36例,侵蚀性葡萄胎14例.结果 按部分性葡萄胎、完全性葡萄胎和侵蚀性葡萄胎顺序,E-cadherin阳性检出率分别为88.0%(22/25)、69.4%(25/36)和35.7%(5/14),表达依次降低,差异有统计学意义(χ2=11.539,P=0.003);Laminin阳性检出率分别为84.0%(21/25)、72.2%(26/36)和28.6%(4/14),表达依次降低,差异有统计学意义(χ2=12.648,P=0.002).结论 免疫组化法分析E-cadherin和Laminin可以作为不同类型葡萄胎传统诊断方法的辅助手段.

Abstract:Objective To investigate the role of E-cadherin and Laminin in the differential diagnosis of partial hydatidiform mole (PHM), complete hydatidiform mole (CHM) and invasive hydatidiform mole (IHM). Methods From January 2005 to March 2016, 75 cases of hydatidiform mole tissues were collected in Department of Gynaecology, the First Affiliated Hospital of Xinjiang Medical University. Hydatidiform mole tissues were paraffin embedded. E-cadherin and Laminin expression were investigated by immunohistochemically SP methods in paraffin-embedded tissue sections, including diagnosed cases of PHM (n=25), CHM (n=36) and HA (n=14). Results From PHMs to CHMs to IHMs, posi-tive detection rate of E-cadherin were 88.0%(22/25), 69.4%(25/36) and 35.7%(5/14) respectively, with statistically sig-nificant differences (χ2=11.539, P=0.003). Positive detection rate of E-cadherin were 84.0%(21/25), 72.2%(26/36) and 28.6%(4/14) respectively, and there were statistically significant differences (χ2=12.648, P=0.002). Conclusion Immu-nohistochemical analysis of E-cadherin and Laminin expression could serve as a useful adjunct to conventional methods in the differential diagnosis of PHM, CHM and IHM.
