摘    要:
目的:探讨化疗药物对心脏损伤的心电图Q-T离散度(Q-Td)及校正的Q-T离散度(Q-Tcd)的影响。方法:以化疗后心电图出现ST-T改变为观察组(n=40),化疗后心电图没有出现ST-T改变为对照组(n=40),比较两组化疗前后心电图Q-Td和Q-Tcd的变化。结果:两组化疗前心电图Q-Td和Q-Tcd值无明显差异(P0.05);观察组化疗后较化疗前Q-Td和Q-Tcd值均显著延长(P0.05),而对照组化疗前后无显著变化。结论:化疗引起ST-T改变提示化疗前患者可能有冠状动脉病变或冠状动脉储备功能不良,该类患者Q-Td明显增加,提示其恶性心律失常或不良心脏事件的发生可能增加,应引起关注。

Abstract:
Objective: To evaluate the effect of QT deviation degree (Q-Td) and heart-rate-corrected QT deviation degree (Q-Tcd) of myocardial injury caused by chemotherapy in patients with malignant tumor. Methods: 40 patients with ST-T change after chemotherapy as observation group and 40 patients without ST-T change as control group were enrolled into the study. Q-Td and Q-Tcd were measured and analyzed before and after chemotherapy in the two groups. Results: There were no significant differences in Q-Td and Q-Tcd between the two groups before the chemotherapy (P 〉 0.05). In observation group, Q-Td and Q-Tcd were significantly prolonged after chemotherapy than those before chemotherapy (P〈0.05), while there were no significant changes in the control group. Conclusion: ST-T changes occurred after chemotherapy may mean the cases suffered from coronary artery disease or worse reservation function of coronary artery. At the same time, Q-Td prolongation induced by chemotherapy may increase the incidence of malignant arrhythmia or worse cardiac events, which should be taken into account.
