Changes of serum retinol binding protein 4 in patients with type 2 diabetes mellitus and its diagnostic value for diabetic nephropathy

Objective To discuss the changes of serum retinol binding protein 4(RBP4)in patients with type 2 diabetes mellitus(T2DM)and its diagnostic value for diabetic nephropathy(DN).Methods A total of 300 T2DM patients were divided into normal albuminuria(N-UAlb)group,microalbuminuria(M-UAlb)group and large albuminuria(L-UAlb)group.100 cases per group according to the ratio of urine microalbumin(UMA)/urine creatinine(UCr),with 100 cases in each group.Another 100 healthy volunteers were selected as the normal control(NC)group.Enzyme-linked immunosorbent assay(ELISA)was used to detect serum RBP4 levels of all subjects.The age,diabetes course,body mass index(BMI)of all subjects were recorded,and UMA,UCr,triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),glycosylated hemoglobin(HbA1c),serum creatinine(Cr),fasting blood glucose(FBG),fasting insulin(FINS),homeostasis model assessment-insulin resistent(HOMA-IR),estimated glomerular filtration rate(eGFR)of all subjects were tested and compared.Results The RBP4 of N-UAlb group,M-UAlb group and L-UAlb group were(47.36±7.23),(62.14±10.02)and(79.38±15.73)μg/ml,which were all higher than(16.25±1.90)μg/ml of NC group,and the difference was statistically significant(P<0.05).With the development of DN,the levels of HbA1c,TG,Cr,HOMA-IR in each group increased gradually,and the EGFR level decreased gradually,the difference was statistically significant(P<0.05).Serum RBP4 levels were positively correlated with UMA/UCr,HOMA-IR,HbA1c,TG and Cr levels(r=0.455,0.497,0.346,0.396,0.385;P<0.05),and negatively correlated with eGFR levels(r=-0.536,P<0.05);and had no correlation with other indicators.Conclusion RBP4 is involved in the occurrence and development of DN,which is helpful for the early diagnosis and monitoring of DN.
