脑梗死患者尿酸、氧化应激水平与早期病情进展关系及普罗布考干预作用研究

摘    要:
目的探讨脑梗死患者早期病情进展与氧化应激、尿酸水平的相关性及普罗布考的干预作用。方法选择2015年6月至2016年5月保定市第一中心医院收治的160例缺血性脑卒中患者为研究对象。将160例患者随机分为研究组和对照组,每组各80例。研究组患者给予阿托伐他汀钙片联合普罗布考治疗,对照组给予阿托伐他汀治疗。于发病第1天测定两组患者的超氧化物歧化酶(SOD)、总抗氧化能力(TAC)、丙二醛(MDA)等氧化应激指标及尿酸水平,于治疗第1、3、7天应用美国国立卫生研究院脑卒中量表评分评价患者卒中进展情况。结果治疗第1天,两组患者的卒中进展概率比较,差异无统计学意义(P>0.05);治疗第3、7天,研究组卒中进展概率明显低于对照组,差异有统计学意义(P<0.05)。治疗第7天,研究组患者MDA水平较对照组低,而TAC及SOD水平较对照组高,组间比较,差异均有统计学意义(P<0.05)。尿酸与SOD、TAC水平呈正相关,与MDA水平呈负相关;且病情进展患者基线尿酸水平偏低。结论普罗布考可以通过降低脑梗死患者的氧化应激水平降低疾病早期病情进展;较高尿酸水平可降低脑梗死患者早期病情进展概率。

Abstract:
Objective To investigate the correlation of uric acid,oxidative stress and early progression in patients with cerebral infarc-tion and intervention effect of probucol. Methods A retrospective study was performed on 160 patients with ischemic stroke who were received in Baoding No. 1 Central Hospital from June 2015 to May 2016. Patients were randomly divided into the experimental group and the control group,with 80 cases in each group. Patients in the experimental group were treated with atorvastatin combined probu-col,and the others in the control group were treated with atorvastatin. The oxidative stress indicators of superoxide dismutase( SOD) , total antioxidant capacity(TAC),malondialdehyde(MDA),and uric acid levels were measured in patients with ischemic stroke at 1st day. The progress of stroke was evaluated using National Institutes of Health Stroke Scale(NIHSS)in the treatment day of 1st,3rd,7th. Results On 1st day,the progressive rate of stroke between the two groups had no significant difference(P>0. 05). On day of 3rd and 7th,the progressive rate of stroke in the experimental group was significantly lower than that in the control group(P<0. 05). On 7th day,the MDA level was lower,TAC and SOD levels were higher in the experimental group than those in the control group(P<0. 05). Uric acid level was positively correlated with TAC and SOD levels,while which was negatively correlated with MDA level(P<0. 05). The baseline uric acid level of progressive patients with stroke was lower. Conclusion Probucol can reduce the early progressive rate of patients with cerebral infarction by decreasing oxidative stress level and increasing uric acid level.
