早期宫内妊娠超声误诊为宫外孕原因分析

目的回顾性分析初次超声提示宫外孕而1周后复查为宫内妊娠的22例病例的临床资料及超声表现,以期发现有价值的信息,避免将宫内妊娠误诊为宫外孕.方法22例停经妇女,尿妊娠试验阳性,初次超声检查提示宫外孕,1周后复查为宫内妊娠病例的临床资料及超声表现,进行对比分析.结果超声所见:附件非均质性包块22例;陶氏腔积液18例;宫内未见孕囊回声.超声均提示出宫外孕待排,1周后复查均诊断为宫内妊娠.结论如在停经6周内,HCG检查为阳性的病例,虽然发现附件非均质性包块(除外内见胚芽和原始心管搏动者)、陶氏腔积液及宫内未见孕囊又无急腹症表现,不要轻易地作出异位妊娠的诊断,建议1周后复查,或许会发现宫内孕囊,从而避免将宫内早孕误诊为异位妊娠.

Analysis of misdiagnosing reasons of premature intrauterine pregnancy into extrauterine pregnancy

[Objective] To summarize the valuable information and avoid misdiagnosing intrauterine pregnancy into extrauterine pregnancy by retrospectively analyzing 22 correlative cases.[Methods] Clinical records and ultrasonic findings of 22 cases which had the same symptom,being menopausic and urine pregnancy test( )and misdiagnosed into extrauterine pregnancy,were collected and analyzed.[Results] Ultrasonic manifestation showed that the masses with unhomogeneous echo could be detected in all of the 22 cases.18 cases showed free liquid echo in the pelvic cavity.Since no Blast Cyst's echo had been detected,the 22 cases were suspected into extrauterine pregnancy and after one week,all cases were reexamined intrauterine pregnancy.[Conclusions] It is prepositional that the symptoms such as menopause for 6 weeks,HCG( )going with unhomogeneous mass echo(except blastrma and primitive cardiovascular system),liquid echo in pelvic cavity,no intrauterine Gestational Sac and no acute abdominal disfunction were not the adequate evidence to diagnose ectopic pregnancy.To avoid misdiagnosing into ectopic pregnancy,callback of the patient in one weeks might be a secure method for perhaps the intrauterine Gestational Sac could be found.
