摘    要:目的探讨全自动血液分析仪检测血常规的临床复检方法及措施,统计分析血常规检测的复检原因。方法按临床标本的检测流程和方法对6084个门诊和住院标本进行全血细胞分析检测,按我室初步制定的复检规则确定复检标本,进行临床复检工作并完成统计学评估。结果6084例标本中3867例标本可以直接报告,无需复检,2217例标本需要复检(占总标本数的36.44%)。复检标本中观察标本一般性状的占187例(3.07%),需仪器复检的标本495例(8.14%),需显微镜观察评估细胞数量或形态105例(1.73%),需人工镜检分类计数的标本1430例(23.5%)。分析以上复检措施可见,观察标本一般性状复检的符合率为100%,仪器复检符合率为62.3%,显微镜观察评估细胞的符合率为73.1%,人工镜检分类计数的符合率为74.2%。结论制定临床适用的复检规则具有很重要的临床意义,严格而有技巧地执行该规则,可提高实验室的工作效率,为临床提供更准确优质的服务。

Abstract:Objective To explore a proper criteria to review complete blood counts and differential results from automated hematology analyzers, and to estimate review reasons that bring following measures. Methods Review criteria for complete blood counts and white blood cell differential were set up. 6 804 blood specimens were analyzed as clinical samples and were reviewed according to the criteria. Statistics on the data were made. Results 3 867 sample could be reported directly and 2 217 samples needed to be reviewed (36.44%). The sample status of 187 samples needed to be observed (3.07%), 495 samples needed to be reanalyzed by automated hematology analyzers, 105 samples needed to be detected by microscopic examination. 1 430 samples needed to be detected by manual differential counts. And the coincidence that results reviewed by sample status, analyzers, microscopic examination, manual differential counts was 100%, 62.3%, 73.1%, 74.2%, respectively. Conclusions It is very important to set up the criteria to review complete blood counts and differential results from automated hematology analyzers, it can improve efficiency of clinical hematology laboratory and provide more accurate and reliable information to clinicians.
