糖尿病患者肾血浆流量的变化及山莨菪碱治疗的影响

摘    要:
对75例糖尿病患者有效肾血浆流量(ERPF)的变化进行多因素分析,并观察其中30例接受山莨菪碱加糖尿病常规治疗4周后的肾功能改变。结果显示,年龄、病程、高血糖和肾脏微血管病是ERPF变化的主要影响因素;山莨菪碱等综合治疗可双相调节ERPF,同时降低肾小球滤过分数(FF)、尿蛋白和尿白蛋白排泄,而对肾小球滤过率无显著性影响。故认为山莨宕碱能有效改善肾内血流动力学异常及肾功能,使其并用于糖尿病的常规治疗之中有助于糖尿病肾病的防治。

Abstract:
Influential factors of renal plasm flow(RPF)were surveyed in 75 patients with diabetesrnellitus,Thirty cases were treated with anisodamine and conventional antidiabetic drugs for4 weeks .With multivariate analysis ,it was revealed that age ,diabetic duration ,hyperglycemiaand kidney microangiopathy were the main factors influencing RPF in these diabetics. Afteradministration of anisodamine and antidiabetic drugs,the lower as well as the higher RPF ofthese diahetics were both restored to nornial level.Glomerular filtration fraction,urinary pro-tein and albumin excretion rates were significantly reduced during the therapy.Thus,aniso-damine may be effective in the improvement of intrarenal hemodynamics and kidney functionin diabetics,and anisodamine combined with conventional antidiabetic therapy may retard theprogression of diabetic nephropathy.
