结核感染T细胞斑点试验在结核性腹膜炎诊断中的临床研究

目的探讨T-SPOT-TB检测在结核性腹膜炎中的诊断价值。方法对我院28例临床和影像学可疑结核性腹膜炎的患者的腹腔积液实施T—SPOT-TB检测，对照组为30例无结核原发病灶的腹腔积液患者。结果28例可疑结核性腹膜炎患者中，26例T—SPOT．TB检测阳性（92．9％），对照组4例阳性（13．3％）。T—SPOT-TB检测诊断结核性腹膜炎的敏感性为92．9％，特异性为86．7％，阳性预测值为82．1％，阴性预测值为95．2％。结论T—SPOT．TB检测是一种具有较高敏感性和特异性的检测结核感染的技术，对快速而准确地诊断结核性腹膜炎有较高的价值。

The Value of T-SPOT. TB Test for the Diagnosis of Tuberculous Peritonitis.

Objective Assess the value of T-SPOT. TB test in tuberculous peritonitis diagnosis. Methods Ascites sam- ples were got from 28 suspected tuberculous peritonitis patients according to clinical diagnosis and imaging analysis, and 30 ascites patientswith no tuberculosis. These samples were analyzed with T-SPOT. TB test. Results 26 suspected tuberculous peritonitis pa- tients were demonstrated positive, with the positive rate 92. 9%. Only 4 ascites patients with no tuberculos is were demonstrated positive, with the positive rate 13.3%. These data demonstrated that the T-SPOT. TB test was valuable in tuberculous peritonitis diagnosis, with the sensitivity of 92. 9%, the specificity of 86. 7%, positive prediction rate of 82. 1% and negativity prediction rate of 95.2%. Conclusion T-SPOT. TB test was sensitive and specific in tuberculous patients diagnosis.
