解放军总医院静脉用药调配中心不合理医嘱分析

目的：通过对静脉用药调配中心（PIVAS）不合理医嘱进行分析,以提高临床用药安全性、合理性和有效性。方法：对解放军总医院PIVAS 2012年8-12月全部14个科室37个病区的医嘱进行审核和统计分析。结果：共审查医嘱373 974组,不合理医嘱组数为8813组,不合理医嘱发生率为2.36%。结论：药师应立足于药房和PIVAS,充分利用自己的专业技能,为临床提供更加可靠的用药信息,保障病人安全和合理用药。

Analysis of irrational medical orders of pharmacy intravenous admixture service in the General Hospital of PLA

Objective： To analyze the irrational medical orders of pharmacy intravenous admixture service（PIVAS）, so as to improve safety, rationality and efficacy of medications. Methods： The medical orders of PIVAS including 37 wards of 14 clinical departments in the General Hospital of PLA from August to December in 2012 were reviewed and analyzed. Results： A total of 373 974 cases of medical orders were reviewed, of which 8813 cases （2.36%） were irrational. Conclusion： The pharmacists in dispensaries and PIVAS should firmly base themselves on their working posts and take full advantage of the professional skills to provide more reliable information to physicians, so as to ensure safe and rational use of drugs.
