| 心脏瓣膜疾病再次手术的临床研究 | |

| 摘 要： | 目的分析因二尖瓣闭式扩张、瓣膜成形术、瓣周漏患者再次瓣膜手术的方法和疗效。方法实施心脏瓣膜再次手术182例。其中二尖瓣闭式扩张手术再狭窄130例,瓣周漏24例,机械瓣功能障碍12例,人工瓣膜心内膜炎16例。再次手术方式有二尖瓣置换、二尖瓣和主动脉瓣双瓣膜置换、主动脉瓣置换。两次手术间隔时间2~23年。结果全组手术后死亡17例,占9.3%。早期死亡主要原因为术后低心排、恶性心律失常、多脏器功能衰竭和肾功能衰竭。结论瓣膜再次手术风险包括急症手术、术前心功能差、合并其他脏器功能不全、体外循环时间长等。针对以上因素采取积极措施可以进一步降低这类患者的手术死亡率。 |
