D-二聚体、P选择素及超敏C反应蛋白联合检测在2型糖尿病肾病中的诊断价值

摘    要:目的:探讨D-二聚体、P选择素及超敏C反应蛋白(hs-CRP)3种指标的联合检测对糖尿病肾病(DN)早期诊断的价值及意义。方法:根据尿白蛋白排泄率(UAER),将125例2型糖尿病患者分为单纯糖尿病组(SDM组)、早期糖尿病肾病组(EDN组)和临床糖尿病肾病组(CDN组);选取44例健康者作为对照组。分别测定各组中D-二聚体、P选择素及超敏C反应蛋白含量。结果:各组组间D-二聚体、P选择素及超敏C反应蛋白水平差异有统计学意义(F=160.32,F=94.17,F=40.89;P<0.01)。随着病情的进展,D-二聚体、P选择素及超敏C反应蛋白含量逐渐增高,以CDN组D-二聚体、P选择素及超敏C反应蛋白含量为最高。D-二聚体、P选择素及超敏C反应蛋白水平分别与UAER呈显著正相关。结论:D-二聚体、P选择素及超敏C反应蛋白水平与DN的发生、发展密切相关,联合检测对DN的早期诊断具有重要意义。

The value of allied examination of D-dimer P-selection and high-sensitivity C-Reactive Protein for early diagnosis of type-2 diabetic nephropathy (DN)

Abstract:Objective: To learn value of allied examination of D-dimer P-selection and high- sensitivity C-Reactive Protein for early diagnosis of diabetic nephropathy(DN). Methods: According to the results of urinary albumin excretion rate(UAER), 125 cases of type, were divided into simple diabetic mellitus(SDM), early diabetic nephropathy(EDN) and clinical diabetic nephrolathy(CDN), other 44 healthy lersons were assigned as control groupl D-dimer, P-selection andhigh-sensitivity C-Reactive Protein of all groups were examined. Results: There was significant difference of D-dimer, P-selection and high-sensitivity C-Reactive Protein in three groups(F=160.32, F=94.17, F=40.89, P 〈 0.01). Moreover, with the progression of disease, the contents of D-dimer P-selection andhigh-sensitivity C-Reactive Protein were increasing gradually. The contents of D-dimer P-selection and high-sensitivity C-Reactive Protein were the highest in CDN group. D-dimer, P-selection and high-sensitivity C-Reactive Protein were positively correlated in straight line With UAER(R=0.742, 0.751, 0.792). Conclusion: D-dimer, P-selection and high-sensitivity C-Reactive Protein are closely related with the genesis and development of DN. Allied examination is very important for DN early diagnosis.
