急性脑卒中偏瘫患者早期康复介入与并发症关系的临床研究

目的探讨急性脑卒中偏瘫患者早期康复介入与并发症的关系。方法选取2008年4月至2011年12月急性脑卒中患者患者220例,分为研究组和对照组,并分别评定治疗第1天、第14天及第28天的肢体运动功能(Fugl-Meyer)以及Barthel指数(BI)分数。两组患者均进行常规治疗,研究组同时给予康复训练。分析早期康复介入与临床运动功能评分、日常生活活动能力、并发症的关系。结果研究组Fugl-Meyer以及BI分数在第14天和第28天与对照组比较明显提高,差异有统计学意义(P<0.05)。研究组在第1天,第14天,第28天运动功能Fugl-Meyer评分与同时期ADL的BI评分有相关性(r值分别为0.59、0.63和0.58,P<0.01)。研究组与对照组比较并发症(呼吸道感染、泌尿系感染)较少,差异有统计学意义(P<0.05)。结论早期的康复介入能够促进偏瘫患者肢体功能的恢复,提高日常生活活动能力。接受早期的康复介入能够明显减少并发症的发生。未接受康复治疗的患者运动功能恢复慢,而且易患并发症。

Study on relationship between early rehabilitative intervention and complications in hemiplegia patients with acute cerebral stroke

Objective To explore the relationship between early rehabilitative intervention and complications in hemiplegia patients with acute cerebral stroke.Methods A total of 220 patients with acute cerebral stroke during April 2008 to December 2011 in this hospital were divided into two groups,control group and rehabilitation group,they were separately judged with Fugl-Meyer scores and Barthel index（BI） at 1st,14th,and 28th days.Patients in rehabilitation group were treated with comprehensive rehabilitation regimen according to physician＇s project beside routine treatment.The relationship between early rehabilitative intervention and Fugl-Meyer scores,BI and complications in these 2 groups were compared and analyzed.Results Motor function and activities of daily living in rehabilitation group at 14th day and 28th day had significantly improved compared with those of control group,their difference was statistically significant（P0.05）.There was positive correlation between Fugl-Meyer scores and BI at 1st,14th and 28th days of rehabilitation group（r= 0.59、0.63、0.58,P0.01）.Complications such as respiratory infection and urinary infection in patients of rehabilitation group were significantly decreased（P0.05）.Conclusion Early rehabilitative intervention can promote the rehabilitation of limb function and improve activities of daily living in recovery of patients with hemiplegia.Early rehabilitative intervention can significantly reduce the incidence of complications.The recovery of motor function is slower in patients of control group and they are also easy to have complications.
